Community Health Programs is looking for caring professionals to join our team! They currently have two positions open:

RN Family Navigator (Full Time)
The RN Family Navigator provides intensive, patient-centered care coordination, health education, and social support to promote improved health outcomes. The RN-FN may work with pregnant women and teens with medical or social risk factors for poor health outcomes, assist with other client populations as contracted by Medicaid Managed Care, or as directed by Family Navigator Program manager. Licensed Practical Nurse (Part Time)
This position will provide nursing support and health education to include focused health assessments in community health screening, childhood vision and hearing and immunization clinics on a regular basis and other clinics as needed.  This job requires the ability to work as a team member of AHEC/Community Health Programs and to provide support for coworkers as needed. Human Services Program Consultant 

The new Office of Health Opportunity (OHO) at the Ohio Department of Health is hiring a Human Services Program Consultant to coordinate statewide comprehensive health equity efforts, including embedding equity considerations and approaches in all Ohio Department of Health program and support offices, coordinating and supporting equity improvement activities at the local level, as well as supporting and coordinating federal equity-related policy initiatives and funding opportunities.   Please share with your partners.
